Tip: If you don't have all of these items, don't worry! We can still see your child and help you obtain missing records. Having them ready just makes the process smoother.
We work with most major insurance plans to make care affordable
You should always check with your Privia Medical Group provider to verify plan and product coverage as this is not an exhaustive list, and plans change frequently. This list will be updated from time to time as needed.
Important: We do NOT accept Medicaid or Florida KidCare
Payment is due at the time of service. For out-of-network patients, deposits may be required.
Insurance Verification: We recommend calling your insurance company to verify coverage and any referral requirements before your first visit.
